What to Prepare Before Starting Your Application

Having the right information ready makes the process faster and reduces the chance of a delay or denial.

Proof of Program Participation

A benefit letter, EBT card, or official document showing you are enrolled in Medicaid, SNAP, SSI, or another qualifying program.

Income Documentation

If qualifying by income, gather recent pay stubs, a tax return, or an employer statement. These show your household meets the 135% Federal Poverty Guidelines threshold.

Address Verification

A lease, utility bill, or official mail with your name and residential address. Lifeline sends the phone to this address, so ensure it is current.

Personal Identification

A state ID, driver's license, or other government-issued identification to verify your identity during the application process.

Household Declaration

You will confirm that no one else in your household already receives a Lifeline benefit. Duplicate household claims are the most common reason for denial.

ACP Program Ended June 2024

The Affordable Connectivity Program (ACP) has ended. The Lifeline Program is the current active federal benefit providing free phone service to eligible households.

  • One free phone per household (not per person)
  • Annual recertification required to maintain benefits
  • You must use the service at least once every 30 days
  • Cannot combine Lifeline with another free phone benefit
