Before you apply
Having the right information ready makes the process faster and reduces the chance of a delay or denial.
A benefit letter, EBT card, or official document showing you are enrolled in Medicaid, SNAP, SSI, or another qualifying program.
If qualifying by income, gather recent pay stubs, a tax return, or an employer statement. These show your household meets the 135% Federal Poverty Guidelines threshold.
A lease, utility bill, or official mail with your name and residential address. Lifeline sends the phone to this address, so ensure it is current.
A state ID, driver's license, or other government-issued identification to verify your identity during the application process.
You will confirm that no one else in your household already receives a Lifeline benefit. Duplicate household claims are the most common reason for denial.
If you want to keep an existing number, have it handy. Most providers support number porting so you do not lose your contacts.

You may qualify if you participate in any of these government programs OR meet income requirements.
Even without program participation, you may qualify if your household income is at or below 135% of the Federal Poverty Guidelines. For a single person, that's approximately $19,000/year. For a family of four, approximately $39,000/year. See full income limits →
